Horizons

Every year, more than 18,000 people in Georgia die from cancer. A considerable percentage of those lives could have been saved with timely cancer screenings, early detection, and effective treatment – but these lifesaving practices are often out of reach.

For uninsured and underinsured Georgians, the cost of screenings and treatment makes them unattainable. For Georgians in underserved parts of the state, simply getting to a screening or testing site may be next to impossible. For all Georgians, investing the time into a cancer screening when no physical symptoms are present may not seem like a priority.

Breast and Colorectal Cancer Screenings For All

Essential to our mission of increasing access to vital healthcare screenings is our Horizons Screening Navigation, in which our Patient Navigators help uninsured and underinsured patients get access to no-cost/low-cost breast and colorectal cancer screenings. Each year, we are proud to see increases in the number of our patients who have had these essential procedures – and we won’t stop until we reach 100%.

A Nationally Recognized Model

Since 2002, our dedicated team has worked to develop a navigator-centric, community-based model for increasing cancer screening rates that has been recognized as an evidence-based initiative for cancer screening nationwide. Together with our provider partners and research partners, we have developed Horizons Screening Navigation into a high-impact, results-oriented program that has demonstrably saved lives in our communities in our region.

Patient Spotlight

Johnny’s Story of Hope

Johnny was a patient at the Albany Area Primary Health Care’s Dawson Medical Center, referred to Horizons for a primary colorectal cancer screening. His FIT test was positive, and Johnny’s next step was to undergo a colonoscopy for a more accurate diagnosis. Unfortunately, this was all happening during the height of the COVID-19 pandemic.

Nonessential procedures were delayed, putting Johnny’s vital screening on hold – and then the pandemic cost Johnny his job. Now uninsured, Johnny turned to his Horizons Patient Navigator, who was able to schedule him for a no-cost colonoscopy at Phoebe Putney Memorial Hospital.

Now cancer-free, Johnny’s timely screening and detection of his cancer at a treatable stage has opened a future that would have never been possible without the work of Horizons and our dedicated healthcare partners.
