Since 2002, Horizons has provided uninsured and underprivileged members of our communities access to cancer screenings and information, helped cancer patients get to treatment, and offered resources for patients, survivors, and their families.

Our History

In 2002, the Southwest Georgia Cancer Coalition was founded as one of six Regional Cancer Coalitions by the state of Georgia to ensure that all Georgians have access to cancer care resources. Founding members of the Southwest Georgia coalition included Phoebe Putney Health System, Archbold Medical Center, Tift Regional Health System, and South Georgia Medical Center.

Today, we are focused on our core mission of providing critical cancer screening, assistance, and survivorship programs as Horizons, South Georgia’s Cancer Coalition.

Research-Tested Intervention Program (2015-present)

Horizons is one of only 166 programs in the country recognized as an RTIP. Our health navigation and cancer screening program is detailed on the NCI website, along with technical assistance and other resources (such as a training manual) for those interested in developing similar programs.
